To save time at your appointment, we recommend you complete patient/client forms prior to your appointment. However, you are welcome to complete the forms in our office.
Required forms: Intake Form, Consent Form, Notice of Privacy Practices (HIPAA), Information Form, Consent for Treatment of a Minor (if under 18):
Information Form (complete the appropriate form):
- Adult Information Form (for ages 18 and up)
- Adolescent Information Form (for ages 12 -17)*
- Child Information Form (ages 8 -12)*
*Consent for Treatment of a Minor (for new clients under 18 years of age)
